We are happy to once again offer Photo IDs to all enrolled families for this school year. Some of you had your photos taken at the family meeting last week, but for those who were not able to do so, below is helpful information for you.
These IDs are NOT mandatory, but are offered to you as a free service. Some people need school IDs to obtain specific forms for their children (i.e. replacement social security cards, recreational or sports program enrollments), and others use them to get discounts at stores, museums, etc. Please read the following information carefully and decide if you need and/or want IDs for your students and family for the upcoming school year. Then follow the steps below:
* If you wish to have a student ID made for any or all of your students, and either teacher (parent), please complete the attached form and submit it to me by email.
*Along with the form, attach a photograph of each person for whom you are requesting an ID card to the same email. It is very helpful if you can name each photo file the name of the person in the photo. Required photographs must be front-facing, non-blurry, plain background pictures of each student and/or teacher. In other words, the photos should be appropriate for any standard, passport- photo ID. Photos must be in .jpg or .png format and no larger than 3MB.
*Please do not submit your photos and form in reply to this email, but create a new email with subject "ID cards" and submit them to sccsmelisha@gmail.com
*The deadline for this school year for submitting photographs and the form via email (sccsmelisha@gmail.com) is Monday, August 22nd. If you wish to receive an ID for this school year, I must receive your completed form and photos no later than the end of the day on August 22nd. ID Cards will be mailed by September 15 to the address each family indicates on your completed request form. If you email your request, make sure that you include the form with your current, correct mailing address. If it is not included, we will mail your cards to the address listed in the enrollment database for the school. Please verify that your address is correct by logging into your account on the SCCS website and checking your information.
